Please read this form prior to your reservation. You will find our Rules, Restrictions, Release & Wavier Form with the button below. You can print a copy or there will be a copy to sign when you come for your reservation. This form is good for the life of the pet listed on the form or until there are amendments to the document.

 

Rules, Restrictions, Release & Wavier Form 2022


Restrictions
1. We do NOT accept aggressive dogs.
2. We do NOT accept dogs with anxiety.
3. We do NOT accept dogs with escape, climbing, digging or destructive tendencies.
4. We do NOT accept intact females that are "In heat/In Season".
5. We do NOT accept any sick dogs.
6. We do NOT accept excessive barkers.
7. We do NOT accept pets that are incontinent or not potty trained.
8. We do NOT accept pets that have been to other pet gathering locations within 14 days prior to their
reservation. (ex. Training Classes, Other Kennels, Day Cares, Vet Clinics, Grooming, Dog Parks, Stores,
etc.)


Rules
9. Your pet must be current on vaccinations (Rabies, Distemper, Bordetella) *note* 14-day policy from pet gathering locations includes Vet Clinics. Vaccinations done with-in the 14-day period prior to your
reservation will not be accepted.
10. You are required to bring your own food. We ask that you portion out each feeding into individual ziplock bags and properly labeled with instructions/information.
11. Soft & crunchy treats are allowed, however, bones & chews are not, as they are potential choking hazards.
12. Do NOT bring your own blankets, beds, bowls, toys. These will not be accepted due to the risk of bugs (ex. fleas, bed bugs, mites, etc.) We will provide alternative options.
13. You are NOT allowed to pet, touch, feed, treat the other pets in the building.
14. You are NOT allowed to open any doors, gates or kennels inside the building. Do NOT insert your pet into a play area or remove your pet from the facility without an employee of Bob's Animal Friends.
15. All pets must be on leash when entering and exiting the property of Bob's Animal Friends.
16. (1) One client/family is allowed in the building at a time. If another family is picking up or dropping off their pet, please wait your turn in your car until they have completely finished. If you are in a hurry, call the shop at 507-635-3398 to let the front desk know you have arrived and need assistance.
17. Bob's Animal Friends is located on our personal property. The family house and out buildings are off limits. The kennel building is marked with a metal sign on the fence. Parking spots are clearly marked and there is a concrete path to the kennel building for you to follow.


Release/Wavier
18. I acknowledge, I am the current and legal owner of ths pet (s) listed on this form and solely assume all risk, dangers, damage and responsibilities that may occur to/or from said pet (s) while attending Bob's Animal Friends. Therefore, I hereby voluntarily release, discharge and agree to hold harmless and indemnify the kennel, it's agents, successors, heirs and employees from any and all liability, claims, demands, actions, which are related to or arise from the services provided from Bob's Animal Friends.
19. I understand, that while staying at Bob's Animal Friends, there are potential risks to my pet (s). Risks can include but are not limited to exposure to viruses, germs, parasites, injury or even death. Bob's Animal Friends has implemented proactive procedures, guidelines and rules to help better protect the well-being of all pets from potential risks.
20. I understand, that not all risks, events or situations can be listed. If I have any questions or concerns, I will contact Bob's Animal Friends or seek advice from reliable resources (ex. Local Vet Clinic). Most common risks can be, kennel cough, puppy warts, fleas, fatigue, hotspots, dehydration, stress, bumps/lumps, scratches, cuts, bites, slips/falls, muscle injuries, acl/ccl tears, parasites, germs, viruses and more.

21. By admitting my pet (s) to Bob's Animal Friends, I am representing that my pet (s) is/are in good health and is up to date on all necessary vaccinations.
22. I confirm, that my pet (s) do NOT have any condition or have been exposed to potential risks within the last (14) fourteen days prior to their reservation/appointment.
23. I understand, that Bob's Animal Friends is a business and there are other family pets' health to consider and
not just my own. That means if your pet (s) propose a threat or concern to another pet's health or well-being you will be requested to retrieve your pet (s) at a moment's notice.
24. I understand, that even if my pet (s) is/are vaccinated for bordetella, which is also linked and referred to as kennel cough, there is still a possible risk/chance of that pet (s) still contracting it. The vaccine your pet (s) receive at the vet is for a specific strain and understand there are other sources that produce kennel cough, respiratory infection symptoms/illness.
25. To my knowledge, my pet (s) have never caused any harm to any person (s) or pet (s) in their life.
26. I will disclose, any and all concerns, health, medical, behavioral, dietary, allergic, and special needs that your pet (s) has/need.
27. I understand, that all services, medical and damages will be paid at the time of pickup / departure.
28. I understand, Bob's Animal Friends has the right to refuse admittance or call for dismissal of any pet (s) at any time.
29. I agree, if any term (s) of this agreement is held by a court to be invalid, void or unenforceable, the remainder of the agreement shall remain in force and shall not be affected or invalidated.


By signing below, I acknowledge that I have read the foregoing Rules/Restrictions/Wavier/Release and understand its contents and fully competent to give my consent: That I have been sufficiently informed of the risks involved and I am giving my voluntary consent in signing it as my own free act and deed.
